    Where can I get an export certificate online to comply with a foreign import requirement?

    Generally, export certificates are not available on line. The standard export certificate is FSIS Form 9060-5 and this certificate must be requested from the inspector in the plant. This form is a four page carbon set and must be either typed or disassembled and printed in a computer printer from a template form designed by the plant for that purpose. Hand written forms are often not acceptable to the importing countries. Some countries require additional forms and most of these are not available online and must be requested from the inspector. Some certification is place on FSIS blank letterhead for lack of space in the 'Remarks' section of the certificate. For convenience, FSIS has placed many of the sets of certification statements on electronic FSIS letterhead stationary and these are found in the documentation section of specific export requirements and are available for downloading.
